Sex

When you do your fitness test at the start of the program, your result is compared with norms for your sex.

Pre-exercise screening

The fitness test is an all-out effort, so please answer these honestly. A "yes" doesn't stop you registering: it means seeing your GP (or, for an injury, a physio or GP) before you do the fitness test or start training.

  1. 1. Has a doctor ever told you that you have a heart condition, or have you ever had a stroke?

  2. 2. Do you ever have unexplained pain or discomfort in your chest, at rest or during exercise?

  3. 3. Do you ever feel faint or dizzy, or lose your balance, during exercise?

  4. 4. In the last 12 months, have you had an asthma attack that needed immediate medical attention?

  5. 5. If you have diabetes (type 1 or 2), have you had trouble controlling your blood sugar in the last 3 months?

  6. 6. Have you been told that a muscle, bone, tendon, ligament or joint problem could be made worse by exercise?

  7. 7. Do you have any other medical condition that could make hard exercise dangerous for you?

  8. 8. Are you currently injured, or returning from an injury, that affects your running?

Informed consent and waiver
conditiond free team sport off-season program: informed consent, risk warning and release This program is provided by Conditiond App (ABN 24 231 575 147), an Australian registered partnership ("conditiond"). It is designed by Nathan Heaney (The Conditioning Consultant). Please read this carefully. By ticking the box you agree to it. 1. About the program • The program is a free, four-week conditioning program for adult team-sport athletes, delivered through the conditiond app. • It starts with a maximal fitness test chosen from: a time trial over 1.6–2 km, the Yo-Yo Intermittent Recovery Test Level 1, the 1.2 km Shuttle Run (Bronco) or the 30-15 Intermittent Fitness Test. • Your training paces are calculated from your own test result. • You do the test and every session yourself, at a time and place you choose. Nobody from conditiond supervises or watches you. 2. Your health • You are 18 or over. • You have answered the pre-exercise screening questions honestly. • If you answered "yes" to any of them, or if your health changes during the program, you will see your GP or physio before doing the fitness test or any session. • The program is general conditioning guidance. It is not medical advice, and it is not tailored to any medical condition or injury. 3. The risks Hard running, including maximal fitness tests and high-intensity intervals, carries risks. They include: • muscle, tendon, ligament and joint injuries; • falls, and injuries from the surface, the weather, other people, vehicles or the place you train; • dizziness, fainting and heat illness; • in rare cases, heart attack, stroke or death. 4. Your responsibilities You agree to: • choose a safe place to train; • warm up, wear suitable footwear and drink enough fluids; • train within your own limits; • stop immediately and seek medical help if you feel chest pain or discomfort, severe breathlessness, dizziness, faintness or pain that worries you. You can reduce or skip any session. 5. Taking part is your choice You take part voluntarily and you accept the risks described above. 6. Release • To the extent the law allows, you release conditiond, its partners, staff and contractors, and Nathan Heaney, from liability for any death or personal injury (including illness) arising from your participation. • This release does not apply where the death or injury is caused by conditiond's gross negligence or reckless conduct. 7. Your rights under the Australian Consumer Law Nothing in this document excludes, restricts or modifies any right or remedy you have under the Australian Consumer Law, or any other law, that cannot lawfully be excluded, restricted or modified. 8. Victorian statutory warning WARNING UNDER THE AUSTRALIAN CONSUMER LAW AND FAIR TRADING ACT 2012 If you sign this form, you will be agreeing that your rights to sue the supplier under the Australian Consumer Law and Fair Trading Act 2012 are excluded, restricted or modified in the way set out in this form, if you are killed or injured because the services provided were not in accordance with the statutory guarantees outlined below. Under the Australian Consumer Law (Victoria), several statutory guarantees apply to the supply of certain goods and services. These guarantees mean that the supplier named on this form is required to ensure that the recreational services it supplies to you- • are rendered with due care and skill; and • are reasonably fit for any purpose which you, either expressly or by implication, make known to the supplier; and • might reasonably be expected to achieve any result you have made known to the supplier. Under section 22 of the Australian Consumer Law and Fair Trading Act 2012, the exclusion of these statutory guarantees is brought to your attention by this form. NOTE: The change to your rights, as set out in this form, does not apply if your death or injury is due to gross negligence on the supplier's part. Gross negligence, in relation to an act or omission, means doing the act or omitting to do an act with reckless disregard, with or without consciousness, for the consequences of the act or omission. See regulation 5 of the Australian Consumer Law and Fair Trading Regulations 2012 and section 22(3)(b) of the Australian Consumer Law and Fair Trading Act 2012. 9. Your information Your screening answers are health information. conditiond collects them only to run the program safely, stores them in Australia (Sydney) and handles them under the program's privacy notice: conditiond.app/offseason/privacy. conditiond's co-founders can see who answered "yes" so they can remind you to see your GP or physio. 10. Agreement By ticking the box you confirm that you have read and understood this document, that you are 18 or over, and that you agree to it. Version offseason-2026-waiver-v2 · 20 October 2026
